Caring for Someone With Dementia? Medicare's GUIDE Program May Be Able to Help
Caring for someone living with dementia can be incredibly meaningful, but it can also be overwhelming. Families often find themselves coordinating medical appointments, managing changing needs, searching for resources and trying to balance caregiving with work, family and their own well-being.
Medicare's Guiding an Improved Dementia Experience (GUIDE) Model was created to provide additional support.
Through GUIDE, individuals living with dementia and their caregivers may have access to care coordination, caregiver education and support, medical resources and, for eligible participants, respite care.
Kore Cares is proud to work with EmpowerMe Medical Group, a participant in Medicare's GUIDE Program, as a GUIDE respite partner, helping eligible families access in-home respite care when they need a break.
What Is the GUIDE Program?
GUIDE stands for Guiding an Improved Dementia Experience.
It is a Medicare program that provides support, education and care coordination to help people living with dementia remain safely at home.
Rather than asking families to navigate dementia care on their own, GUIDE provides a care team that can help coordinate services and connect individuals and caregivers with additional support.
Through GUIDE, individuals living with dementia and their caregivers may receive:
Care navigation: caregiver education, support and check-ins; help connecting to resources and referrals; and care coordination support.
Medical support: telehealth access when additional support is needed and connections to specialty providers when appropriate.
Respite care support: eligible individuals may receive up to $2,625 or 76 hours per year for eligible respite services, which may include in-home respite care or adult day services.
Who May Be Eligible for GUIDE?
GUIDE may be available for individuals with traditional Medicare who have a dementia diagnosis, live at home and are not enrolled in Medicare hospice.
An individual must have a dementia diagnosis, be enrolled in traditional Medicare Parts A and B rather than Medicare Advantage and not reside in assisted living, memory care or a long-term nursing home.
Families do not need to figure all of this out themselves. The GUIDE Care Team conducts a comprehensive assessment to evaluate the individual's medical condition, care needs and situation, as well as the needs and burden of an unpaid caregiver.
One of GUIDE's Most Valuable Supports for Families: Respite Care
Caregiving can be physically and emotionally demanding. Even the most dedicated caregivers need opportunities to rest, attend their own appointments, spend time with other family members or simply recharge.
Respite care is short-term support that gives caregivers time to rest and recharge while their loved one continues to receive safe, attentive care.
For qualifying GUIDE participants, up to $2,625 or 76 hours per year may be available for eligible respite services, including in-home respite care or adult day services.
That respite time might allow a family caregiver to:
Go to their own medical appointment.
Run errands or attend an important event.
Spend time with their spouse, children or friends.
Rest or take care of their own health.
Simply have a few hours when they don't have to be "on."
Taking a break isn't stepping away from caregiving. Rest can help caregivers return with more patience, clarity and energy while reducing the risk of caregiver burnout.

Kore Cares is a GUIDE respite partner working with EmpowerMe Medical Group.
When an eligible GUIDE participant is referred to Kore Cares for in-home respite services, our trained care professionals can step in to provide support for the person living with dementia while their family caregiver gets a much-needed break.
For families already caring for someone at home, that means GUIDE isn't just about providing information or advice. For qualifying participants, it can provide access to real, hands-on respite support in the home.

GUIDE Supports the Caregiver, Too
One of the things we appreciate most about GUIDE is its recognition that caring for the caregiver is an important part of caring for the person living with dementia.
Caregivers often put their own needs last. They postpone appointments, miss activities, lose sleep or feel guilty about taking time for themselves.
But caregiver well-being matters.
GUIDE provides caregiver education, support and check-ins, connections to resources and, when eligibility requirements are met, respite services.
The goal isn't to replace the family caregiver. It is to give families more support so they don't have to manage dementia care alone.
