Nurse Job Description

As a nurse with Kore Cares, your primary responsibility is to provide essential nursing care to clients, following the outlined care plan and ensuring they can maintain residence in their home. This involves a range of tasks such as administering medications, monitoring vital signs, and implementing medical procedures as per the care plan. Your role is crucial in promoting the health and well-being of clients, addressing their specific medical needs, and collaborating with other healthcare professionals to optimize the care provided. By offering nursing services, you contribute significantly to the overall goal of enabling clients to receive necessary medical attention while staying in the comfort of their own homes.

responsibilities and duties include but are not limited to:

  • Assesses the full scope of patient needs using the nursing process on a continuous basis

  • Observes signs and symptoms and reports changes in the patient’s condition to the physician and/or other appropriate health care professionals

  • Provides teaching to client and client family members

  • Delegates tasks to other Kore Cares personnel in accordance with the South Dakota Nurse Practice Act

  • Uses knowledge and judgement needed to meet nursing needs of the patient

  • Nursing tasks may include but are not limited to: wound dressing, medication set up, monitor medication compliance, assess vital signs, assess skin condition, assess nutritional status, assess mobility, assess usage of glucometer, toe/nail care, catheter care, assess health, provide teaching/health education, communicate with physician and pharmacist, blood draws

  • All other duties outlined in client care plan

education and experience:

  • 2 years of nursing experience, preferred

  • Must have active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of SD

required skills and abilities:

  • Must be able to physically perform the duties of the position

  • Must be able to read, write, understand and speak English

  • Excellent verbal and written communication skills

  • Excellent time management skills

  • Excellent organizational skills and attention to detail

  • Must have reliable transportation and current valid driver’s license

  • Proof of automobile insurance and eligibility for employment in the United States
