Care management has been identified as a huge need for not only our current clients but for many individuals in and around the Sioux Falls community. We are continually trying to find ways to not only help individuals with activities of daily living but also connect them with resources to help them achieve the best life possible. That is why we are happy to announce the addition of care management to our service offerings.

The New Year is days away and the holidays are wrapping up. Family is headed back home and ready to move into 2019. Sometimes, if you’re a family member who lives out of town and doesn’t get back as much as usual, you may notice changes that you weren’t aware of the last time you visited. While you were home for the holidays, did you notice your mother/father, grandma/grandpa, etc. seemed to be having a more difficult time carrying out everyday tasks than normal? Maybe they could use a little extra help in the home.

Naturally, as we age, we change both mentally and physically. These changes can present challenges that affect day-to-day life. This doesn’t mean you’re unable to live alone or unable to preform a majority of your daily tasks. It just means that particular everyday functions may become harder to accomplish and present more risks for falls or other injuries.

October 21st-27th is Pastoral Care (also known as spiritual care) Week 2018. The purpose of this week is to bring awareness to pastoral care and all of the benefits it’s caregivers bring to the people they serve. We would like to thank those working in pastoral care for the differences they make in the lives of those they serve!

Thursday, October 18, Kore Cares will be holding the second community education event of the Fall series. Haley Samuelson will be presenting “Introduction to Crucial Conversations for Caregivers”. Professional and Family Caregivers, Home Health Aides, Certified Nursing Assistants, and Direct Support Professionals can learn effective techniques for having difficult conversations.

There are many definitions for care management, but to put it quite simply- it is assistance with coordination of services related to a person’s overall wellbeing. A care manager can help individuals identify paths to improving one’s health and assist them in achieving their health care goals.

People are often asking what kinds of services we offer at Kore Cares. We can list our services, but people who are unfamiliar with home care are often still left wondering exactly what it is they need and for how long or how often they need it. We hope that providing scenarios in this blog post as well as previous blogs will help answer your questions. Our services are customizable to each individual. No client is the same and we don’t expect their needs to be either. No matter what it is, even if it doesn’t end up being our services you need, we are here to assist in any way possible.
